This clinical report shows the long-term results of a rehabilitation of a completely edentulous mandible with a severely resorbed ridge through an ITI hollow-screw implant-supported bar-joint overdenture in a patient who returned for a follow-up 14 years after its installation. The patient was a female patient that had 55 years old at the time of implant placement and showed a completely edentulous mandible and a partially edentulous maxilla. Four 4.1x12mm ITI hollow-screw (Bonefit) implants were placed in the area corresponding to teeth number 34,33,43,44. A gold reinforced bar-joint overdenture using clip-retention was fabricated to rehabilitate the patient. The patient never appeared for a follow-up and returned 14 years after prosthesis installation because its retention had been compromised due to the fracture of the clip. The patient did not show clinical signs and symptoms such as pain, infection or mobility, and no bone loss around the 4 implants supporting the mandibular bar-joint overdenture was observed through panoramic radiograph. The fractured clip was replaced by a new one (CM rider, Cendres & Mentaux SA, Switzerland) and a favorable retention of the overdenture was obtained.
